• Information reasonably sufficient to contact you, such as your address, telephone number, and email address if available.

• A statement that the information in the notice is accurate and, under penalty of perjury, that you are authorized to act on behalf of the copyright owner.
• Your physical or electronic signature.

If you believe material was removed or disabled by mistake or misidentification, you may submit a written counter-notification to our Copyright Agent.

• Identification of the material that was removed or disabled and the location where it appeared before removal.
• A statement under penalty of perjury that you have a good faith belief the material was removed or disabled due to mistake or misidentification.
• Your name, address, telephone number, and a statement that you consent to the jurisdiction of the appropriate court and will accept service of process from the person who submitted the original notice.

If we receive a valid counter-notification, we may reinstate the removed material in accordance with the DMCA and applicable law.
